SELF-REPORTING

Students who receive a positive COVID-19 test are asked to fill out this secure private form to self-report cases. 

Employees who test positive or are advised to quarantine must notify their supervisor and then Human Resources at 605-642-6877.

The South Dakota Department of Health will continue to do contract tracing of COVID-19 cases on campus and will contact people who have been identified as having contact with a positive COVID-19 case.

Students should continue to be alert for symptoms including fever, shortness of breath, and a cough. BHSU is collaborating with Monument Health to test students with symptoms of COVID-19.  Students who are experiencing symptoms, please contact Student Health Services at 605-642-6520 so they can direct you to the free COVID-19 testing.

Faculty and staff who have symptoms should contact their health care provider or the Monument Health Nurse Triage Line at 605-755-1350 or 800-279-1466
