Departmental Inventory Deletion/Change Form

Complete this online form for any inventory item that needs to be removed from the current department's inventory list i.e. inventory relocated, used for the garage sale, recycling, or discarded as trash. Purchase documentation information is required for items with a capital asset tag (original value of $5,000 or more). Submit one item per form.
