Responsibilities for Completing Forms during One Year Residency/PDS

During the first semester (Methods Block)

• Complete a Graduation Check (if not already completed)
• PRAXIS Content Knowledge exam (if not already taken)
• Sign up for PLT Exam
• Attend Sign-Up Meeting for Student Teaching

Student Teaching Handbook

At the beginning of the second semester internship

• Meet with the University Supervisor during the Orientation meeting
• Meet with the Clinical Faculty and University Supervisor to complete the Initial Visit Form and discuss Co-Teaching Strategies

During the second semester internship

• Weekly Conference Form: suggested format for identifying strengths and areas to solidify; these forms are not sent back to the Office of Field Experiences and may be used for conferencing during observation debriefings
• Leave of Absence: to be used for all absences (professional, sick, and personal)
• Lesson Plan Format: to be used for all formal observations
• Attend the PLT Study Session
• PRAXIS PLT exam
• Formative Evaluation of Intern Form: 
  • 3 for a 16-week placement (by the University Supervisor and Clinical Faculty – total 6)
  • 2 for an 8 or 10-week placement (by the University Supervisor and Clinical Faculty – total 4)
  • 1 for a 5-week placement (by the University Supervisor and Clinical Faculty – total 2)
  • Additional as needed
• TWS Due Dates

Around the midterm of the second semester internship

• Professional Dispositions Assessment Form (PDA) self
• Diversity 4.d.1
• Evaluation of the Clinical Faculty (one for each placement)
• Evaluation of the University Supervisor (one for each supervisor)

By the end of the second semester internship

• Internship Recommended Final Grade Report – will be signed by all
• Exit Survey
• Attend the Exit Seminar

• Internship Final Appraisal Form: this is the summative evaluation of overall achievement by the Intern based on formal observations. This is the only document to be shared with school district as reference if requested
• Internship Recommended Final Grade Report – please print and acquire signatures (this needs to be signed by intern, clinical faculty, and university supervisor)
