The Appendix F procedure is established by the Contract COHE has with the Board of Regents. All faculty unit members are evaluated on an annual basis by their immediate supervisor. See BOR Policy 4:13; and BOR/COHE agreement. Individual Development and Educational Assessment (IDEA) surveys are used as part of the evaluation of faculty unit members as required by Section XI of the COHE Agreement.

Before completing the Appendix F the first time, one may want to ask his or her department chair or another faculty member for guidance or to look at a completed Appendix F sometime prior to the beginning of the first of the year. On January 15 of each year or the first working day thereafter, every faculty member is required to submit a completed Appendix F to his/her department chair. The format is presented as follows:
