Complete this Vehicle Request Form in its entirety. You will receive an email confirmation after the vehicle has been reserved. You will pick up your vehicle keys at the Facilities Services Office between our office hours of Monday-Friday 7am-5pm.
NOTE: Student Driver's are now asked to complete the Volunteer Agreement Form for insurance purposes to protect the driver.
