Responsibilities for Completing Forms during Student Teaching Internship

At the beginning of the internship During the internship
  • Weekly Conference Form: suggested format for identifying strengths and areas to solidify; these forms are not sent back to the Office of Field Experiences and may be used for conferencing during observation debriefings
  • Leave of Absence: to be used for all absences (professional, sick, and personal)
  • Lesson Plan Format: to be used for all formal observations
  • Register and take the PRAXIS PLT exam at www.ets.org/PRAXIS; all PRAXIS are computer-based and proctored at various sites (Rapid City, Casper, Billings, etc.)
  • Formative Evaluation of Intern Form: 
    • 8 or more for a 16-week placement (4 by the University Supervisor and 4 by the Clinical Educator – minimum 8)
    • 4 or more for an 8 or 10-week placement (2 by the University Supervisor and  2 by the Clinical Educator – minimum 4)
    • 2 or more for a 5-week placement (1 by the University Supervisor and 1 by the Clinical Educator – minimum 2)

By the end of the internship

Please consider submitting some of the following documents online at least a couple of weeks prior the exit seminar. The office needs to check all submissions to inform you of the missing documents when you come for the meeting.
